General Surgeon 's  Response
Hi.
Thanks for your query.
Noted your detailed history related to hydradenitis suppurativa, operated but seemingly recurred pilonidal cyst, your unwillingness to undergo general anesthesia, enlarged spleen with multiple cysts.
My thoughts:
The enlarged spleen with simple cysts if not causing pain needs no special treatment. If the cysts are complex or suggestive of hydatid disease splenectomy has to be done.
Hydradenitis suppurativa needs excision and skin grafting for cure, no other treatment helps.
Pilonidal sinus if recurrent needs wide local excision and recurrence is common by 30%. So get operated with good faith and you may be cured.
