Suggest Treatment For Fluttering In Chest,dizziness And Nausea

I am 32 years old about 5 4 and 315lbs. I have been experiencing dizziness, nausea, and fluttering in chest. I also feel like I can hear my heartbeat in my ears and it takes my vision to catch up to the way I turn my head. For the better part of the last year I have not felt good. I am on Effexor and linisopril. I have seen my regular doctor a few times explaining this but I m not getting anywhere. I just don t feel well. I m tired all the time. I realize a lot this is weight and I have a hard time even beginning to lose weight but I can t even think to exercise when I can t move my head without feeling faint. I also have a problem with burping all the time and I don t mean after meals or after I drink. I burp all day with a full feeling in my stomach and never any relief. I don t have any pain. I have not had a period in nearly 7 months as well. No I am no pregnant. Please help me. I just want to feel better.

I passed carefully through your recent medical history and would like to explain that dizziness and nausea are nonspecific complaints that need to be carefully investigated in different directions as they may be due to a neurological issue, a cardiovascular, digestive or metabolic disorder.

Considering your symptoms like frequent burping, stomach fullness, it is necessary to first exclude any possible adverse effect of your current medication, Effexor (Venlafaxine), which is well known to produce nausea and doziness in a large proportion of patients (up to 50%).

After excluding this cause, a comprehensive review of the digestive system needs to be done for excluding a possible gastroesophageal reflux, hiatal hernia, and even diaphragm fluttering.

Also, a review of your cardiovascular system should be performed focusing to possible episodic arrhythmia (like paroxysmal supraventricular tachycardia, atrial flutter, etc.). A cardiac ultrasound and Holter monitoring would help in this regard.

In addition, supra-aortic vessels Doppler ultrasound to exclude any possible arterial stenosis or subclavian steal syndrome are recommended.

A neurological physical exam should be done to exclude possible neurological implications.

At the end, I recommend consulting an endocrinologist for properly addressing the issues of obesity and potential endocrine implications (thyroid, adrenal dysfunction).
