Brief:
Fever this high should be diagnosed.

Detailed:

Hi
Wellcome to health care magic.
I am Dr.Muhammad Ahmad and i will be looking into you question,i have read it completely and understand what you are asking.

SHould you be concerned?
Yes at a fever of 104 you should be concerned it can be taken lightly.

POSIBILITIES: Fever this high is usually due to an infection which can range from bactetial to parasites like malaria or rarely a responce to a drug

Is what you have done so far ok?: yes you did it right so far by giving her an ani fever med and cold sponges.

what to do next:
She should have a thorough checkup a diagnosis should be made and medicated accordingly.

What is dr expwcted to do:
If i were you doctor i would have taken a thorough history from you and her ruling out more dangerous stuff like TB,typhoid,dengue.. malaria...urinary tract infectior or any skin infection of hepatitis.

Would have run her baseline tests like
COMPLETE blood count
RFT
LfT and simple urine study

A cause of fever is established in 95% of cases till this stage of clinical proceding then i would have suggested medicine accordingly.

I hope i helped you decide your nex move with reason and options better and in a sequence if you have any further query i am happy to help you. I Wish her quick recovery .
