Hi..
Welcome to HCM.

Grade 2 fatty liver with hepatomegaly in a obese patients usually require treatment along with lifestyle modification. So I would advise,
1.Get Fasting lipid profile which checks amount of cholesterol in body.If high start cholesterol lowering drugs such as atorvastatin/rosuvastatin.
2.Reduce obesity by taking low calorie diet and regular exercise. If BMI>35 she definitely requires medical intervention to reduce obesity.
3.Life style modification like avoiding alcohol/smoking, low calorie diet and diet devoid of cholesterol.
4.Yearly once ultrasound scan of liver to look for progression.
