Suggest Treatment For Enlarged Prostate And Elevated PSA Level

My PSA has risen from 1.5 to 6 in 3 months. I have had an enlarged prostate (digital and ultrasound screening) for 2 years. The only significant history is HCV genotype 1a which was resolved with Harvoni two years ago and an ex-smoker (5 years free). Ancillary to HCV, I had cryoglobulinemia, thrombocytopenia, RA, and rashes etc. All have returned to normal or dissappeared but I have not yet returned to a normal state. My body is still adjusting. I have ED in which I cannot maintain an erection during intercourse. I take viagra for this. I have just been put on a sulfa drug prior to retesting my PSA. Should a biopsy be necessary, what is the probability of worse ED? Can this be corrected. I also cannot ejaculate except into my bladder. Is this correctable? If my prostate is removed, I ask the same questions as above. I also have COPD. I have found that 3L of O2 during intimacy greatly improves a satisfactory outcome for me. I have read of a hepato-pulmonary syndrome and a hepato-pulmonary-gastro syndrome. Could my current problems be a result of my body trying to return to normal after 49 years of HCV and liver damage? I am 67 and was a triage specialist in the service of the US Army Medical Corps attached to the 101st Airborn in 1969. I was very atletic and still run 3 miles a day with O2 strapped to my back. My pulmonologist has asked me to exercise 30 min/day. I work out with cardio/weights one hour/day. Consequently my weight has risen from 170 to 180 in 3 months. I have had an ongoing love affair with my wife of four years. My previous wife died of lung canter in 2012. We were married before I left for Hua. I/we have been very sexually active and would miss this integral part of my life. Thanks, Dave Shrout

These symptoms indicate in most cases prostate issues such as prostatitis or hypertrophy and rarely it may be sign of STD infection. You should avoid intercourse for till symptoms persist and drink plenty of fluids. If symptoms persist it would be good to do prostate ultrasound and check your PSA levels to rule out prostate tumor or hypertrophy and further treatment depends on these findings. In most cases it can be controlled with medications but sometimes, in case of tumor, open surgery is required. And severe benign hypertrophy may be managed endoscopically so you should consult urologist about appropriate options.
