I have gone through your query and found your your medication and your high level of BSL inspite of your medication and control on diet and exercise . It is a matter of great concern, the counts of BSL are indeed quite high .

Diabetes depends on 'what we eat ,when we eat and how we eat . and our activity level . It is a life style diseases and can be controlled if we try sincerely .
Gulping on only tablets and
insulin is not enough and not going to help anyway ,unless A strict diet control and regular exercise to control weight is made part of the therapy.
I suggest you to check your regimen with the regimen given below as it is tested & tried on hundreds of diabetics who have come to normal BSL with minimum medication prescribed by their physician .
Result of following the regimen will be seen after 2-4 months and is to be followed for life .
I suggest you to take balanced regular diet .Include all essential nutrients less carbs , more of fiber,protein , vitamins ,minerals in natural form as fruit ,green leafy veges , supplements full of antioxidants - lemon juice ,bitter gourd juice 25 ml ,
Aloe vera juice +Amla juice , coconut water , bitter gourd juice 25-30 ml , turmeric powder with a cup of hot milk + a T spoon almond oil, almonds ,walnut , fenu greek powder a Tspoon, a few to count , and proper , exercise regular ,walk ,YOGA,pranayam - deep breathing will fulfill your requirement of balanced diet ,and overall
detoxification and strengthen of your system .
You must keep in mind not to include sugar or sugary products , carbs in your diet .
