Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concerns..

As per your complain it seems that severe cuts and burning sensation around anus can be due to causes like anal fissures or hemorrhoids..
Also as the medications given to you that is ebastine tablet and onabet b cream has helped, it is also pointing towards inflammatory conditions only..

I would suggest you to consult a Proctologist or an Ano-rectal surgeon and get evaluated and a thorough clinical evaluation and investigations like anoscopy can help in diagnosis and treatment can be done accordingly..

In case of anal fissures you can be advised to apply topical anaesthetics like Lignocaine gel..
Calcium channel blocker containing ointments like Diltizem can be advised as it will dilate the blood vessels in the area and improve blood flow that will initiate healing..
You can be advised to take stool softeners like Docusate and increase fibre intake that will also smoothen the stool like Psyllium and methylcellulose..
You can also do Sitz bath by sitting in a tub of warm water 2 to 3 times a day especially after bowels..
You can also take painkillers like Ibuprofen if there is pain..

In case of hemorrhoids apply steroid and numbing ointment..
-Sitz bath especially after bowels....
-High fibre diet along with laxatives like Polyethylene glycol and stool softeners like Docusate to maintain proper bowels..
-Avoid spicy food and drink plenty of water..

-In case of no improvement with medical treatment you can be advised surgical procedures like Rubber band ligation, sclerotherapy or Hemorrhoidectomy that is surgical removal of the hemorrhoids..

Hope this information helps..

Thanks and regards.
