General Surgeon 's  Response
Hi.
Thanks for your query.
The appearance of the vein like structure from the anus to the anus towards the scrotum - painful and uncomfortable with a of fluid filled pocket, and you drained it is suggestive of a fistula in ano unless proved otherwise.

I would advise you the following:
First of all stop fiddling with such problems as you may complicate more.
Consult a General Surgeon for clinical evaluation and actual physical examination.
MRI of the area for fistulogram
Get started on appropriate antibiotics, metronidazole and probiotics and anti-inflammatory medicines
Prepare for surgery if you want permanent relief as such tracts are nasty to treat.
