Suggest Treatment For UTI And Fever In A Child

Hi, my 8 year old grandson in Barcelona with my daughter and son in law. Just off cruise ship in Mediterranean. Developed 104.5 fever, checked out at ER with urinalysis and ultrasound; UTI and appendicitis ruled out. They sent him home, no blood work, no chest x-ray. Other symptoms diffuse belly pain, headache, leg pain. Fever temporarily dropped to close to normal, now back up to 104. Daughter says it rises whenever he goes to sleep. Should she just continue to monitor symptoms and treat with Motrin, or alternate Motrin with Tylenol to keep fever under control. They are attempting to move up their flight to tomorrow to get back to US.

Thank you for your query.

A high grade fever that lasts for more than 3 days or reduces and shoots back up should be investigated in detail.

-what other symptoms does he have? Nausea, vomiting, loose stools, yellow discoloration of skin or eye.
-any shivering associated with fever, any rashes?
- did the ultra sound show any liver or spleen enlargement?
I recommend:
- a pediatric consult
- blood tests (complete blood count, liver and kidney function tests, antibodies to dengue virus, leptospirosis, peripheral smear to rule out malarial disease)

In the meantime, keep him well hydrated, a
Keep the temperature under control. Provide bland and easily digestible food. Avoid spicy and oily foods and complex carbohydrates.

If he is lethargic or drowsy or the fever cannot be controlled for more than 24 hrs, or urine output is less, take him to the ER right away.
