Suggest Suitable Therapy For ADHD In A Teenager

I have a 16 year old son diagnosed w ADHD age5 he has taken concertante then vynase 40. Mg He also took Zoloft for a short period of time as the heightened noises on the bus and after day care would have him throwing up nearly every day, the drug was d/c after he mentioned 3 times he felt he might rather not be here anymore. Like end his life. The ongoing exacerbating issue now is his blatenet disrespect for me his mon, he will not listen to what I ask is very defiant, to the point he speaks over me pushing me to my breaking point where I want to smack him I never have but he has hit me once and he is very strong . He can hurts me. Last night was particularly bad as my husband and I are both OR nurses his dad is not home a lot therefore I am left to discipline my son and I can not do this as he has fit of anger and rage. My daughter told me last night that he has hit her over the summer to the point of near uncoinciouness then was later remourseful. She is 14. I have taken him to councelors but not on a continual basis, the death of my father tremendously affected him yet he has always had this dark side where he disrespects myself, my best friend and his sisters to where he has been physical. I am of small stature and think he knows he could easily hurt me. My concern is is he bipolar. .? I have anAunt who is diagnosed with it and truly believe my one sister is mentally ill , mental illness runs rapid in both sides of the family. When I mention going to counseling he becomes very angry and agitated. he says he will not go I have not been able to find a psychiatrist near by nor do I know which type to go see Is the by vynase the wrong drug?

we need more history as there is no blood test or scan that can used to pick up
another risk factor is anphetamine can causes psychotic and. mood disorders
important aspects including his sleep and episodic nature of the problems need to clarified
broad suggestion is children on vynase developing recent onset behavioral disturbance, there is high possibility of drug induced mood disorder
hence my suggestion would be taper and stop vynase under expert guidance and monitor the symptoms
