Suggest Remedy For Multiple Ailments

Dear Doctor,

My mother is 60 yr. old, with history of C.A Breast & mastectomy was done five years ago (Radical mastectomy) and kept on hormonal therapy till now (Tamoxifen tab 20mg 1x1) without chemotherapy or radiotherapy. And regular follow up done every six months and these include: blood test (C.B.P, ESR, Renal function test, liver function test, S. electrolyte, lipid profile), uls (ultrasound) for abdomen & Rt. Breast, CXR and mammography every year.

Last one year, she complains from intermittent backache radiated to unilateral Leg, so plain x-ray & contrast MRI done and it shows degenerative changes of vertebrae, and kept on anti-inflammatory and analgesic drug and improved.

Before four months, she complains also severe backache and radiated to unilateral leg and difficulty in walking describe as (Bone pain), this pain last 20 days after history of strenuos exercise (blood test = normal) and kept on analgesia.

The patient had history of intermittent dizziness and headache, no history of nausea, vomiting or blurred vision.

Three weeks ago, bone scan done, shows multiple focal lesion (skull, ribs & vertebrae)??? But the oncologist don’t depend on this scan, because this is the first machine enter our country, Iraq, so I would like to know your opinion and your advice please….

Note: -The patient had history of trauma to chest before few months ago and complains from chest pain for two weeks and then relieved.

-     Previous history of operation of the left hand (Trigger finger) 8 years ago.

-     This Bone scan done after one hour & 45 Min. from the time of injection of Tech.99 and the scan duration was only 20 minutes.

-     My mother don’t complain any pain since last May 2005.

A known cancer breast patient with bony pain could always have bone metastasis and needs to be proved that they are not cancer... If bone scan is not available elsewhere u can go ahead and do a whole body pet ct scan which will also pick up cancer spread to bone if any is present..

If the treating doctor thinks it is not cancer spread to bone but just age related changes only still she may need biopsy from any of those lesions to be done to prove that they are not cancer spread.

probably biopsy from rib.

but biopsy is an invasive test and do i would suggest a whole body pet scan.

Initial diagnosis pathology report will be helpful to see how aggressive disease was initially.

also an ultrasound abdomen may pick any liver disease if present
I find this answer helpful
