Hello
In the beginning sputum was POSITIVE and anti tubercular treatment started , retested after 5 months , still sputum was POSITIVE i.e. I would not say failure of treatment but ineffective treatment . Continued for another 13 month , no improvement , still doctor advise keep this treatment for two ( in all ).
I have seen so many cases like this with these symptoms and signs.
In my opinion this patient has developed MULTI DRUG RESISTANT TUBERCULOSIS .
Now get in X RAY chest ( for fresh lesions) , sputum for A F B , Blood for C B C ( leucocytosis may present) E S R ( very high > 100 mm) , Protein Purified Derivative test- Tuberculin Skin Test.
If possible get in Bronchoscopy .
Now upload all these reports or consult a CHEST & TUBERCULOSIS specialist and get his opinion.
Hope this will help you.
Good luck.
