Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

As per your query you have symptoms of extreme toothache in spite of taking combination of Ibuprofen and Acetaminophen which seems to be due to exposure of pulp tissue and it should be treated immediately. Need not to worry. I would suggest you to:
• Do warm saline rinses several times a day.
• Avoid hot and extreme cold food intake as it can exaggerate your symptoms.
• Take higher dose of painkiller such as Tramadol.
• Do betadine rinses twice a day.

I would suggest you to consult a dentist for proper oral prophylaxis. Doctor may perform a root canal if the pulp of the tooth gets infected. Doctor may also perform apicoectomy in case of abscess at the tip of the root.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Harry Maheshwari
