Suggest Diet For Diabetic And Obese

Hello! I am a 59 y/o, female with Type 1 Diabetic, obese, post thyroidectomy (CA), CHF (lungs sound good now) and disabled due to bad knees, neuropathy and arthritis.I am considering Lap Band. But I need help now so I can breath eaiser.I am also watching my salt intake and fluids in and out. Is there a diet that takes calories and carbohydrates into a single plan? Who could show me such a diet plan? Summer is coming and I will have access to a pool for cardio exercise. Thanks for your help in advance!
