Based On the facts, You mostly seem to suffer from-
Hydrocele after
Varicocele Surgery- 3 yrs back.The lymphatic and venus obstruction caused after Varicocele surgery has caused this lump from Secondary hydrocele.Hence your left testes seems larger than the right one.The hot feeling with this developed lump comes due to the stretching of the vascular sympathetic supply which is strangled in the Varicocele repair Surgery,which is ligation of the pampiniform plexus.
Recent treatments by Varicocele embolisation / and or endoscopic repair of the testiular vein Varicosity mostly on left side? due to the left renal
drainage of the left testicular vein.(in 85 % cases )
Treatment for -Cure of discomfort in testicle-
Hydrocelectomy would reduce the traction pain on the dis-organised left funicular cord and would reduce the chances of recurrence by inversion of the Tunica Vaginalis with exposure of the secreting surface to the
subcutaneous tissue.
Also the continuous Lymph or Fluid secreted and collected in the testicular sac of Tunic Vaginalis,would not cause recurrent hydrocele.
Thus hydrocelectomy would partially reduce the discomfort in the testical on left side of yours.
Thus you can treat the discomfort by Hydrocelectomy and by micro-vein ligation with preserverance of Lymphatic vessels.
