Suggest Alternative For Minivelle.75

I had a complete hysterectomy 15 years ago and after going through some really weird side effects of ert I ended up taking 1mg of estradiol for all of these years. Seemed to be fine until a few years ago and extreme night sweats began (3-4 a night) which has caused a major inability to sleep through the night. I literally wake every 2 hours but am able to go back to sleep. My MD had suggested I decrease my estradiol because of my age 56 so I varied between .5-1mg. Recently, he switched me to the minivelle .75-.5mg trying to figure out which dose is right for me. The .75 causes painful breasts and the .5 does nothing much. I am EXHAUSTED all the time and my GERD is acting up again which it did when I first began ERT. My GERD is a weird kind where I rarely have reflex. Instead it affects my bronchioles. I m beyond frustrated as I am a very active and energetic person. I just want to sleep and have good energy. I m not overweight and I exercise. Thank you for any help you might be able to give me.

I would have suggested my patient to begin on paroxetine 7.5mg (Brisdelle) for the vasomotor symptoms (increased sweating).
Now regarding continuation of Minivelle, I would be cautious and wouldn’t stop it altogether. You are already on HRT since 15 years and I would not like it to stop abruptly. I would advice to continue Minivelle 0.5mg and slowly taper over months if required.
I can understand your concern about GERD. I would have prescribed a proton pump inhibitor like pantoprazole 40mg once daily before food. Apart from medications, I would advice lifestyle modifications for GERD like avoiding sleeping on right side, strict diet with taking servings at regular intervals, reducing stress.
