Suggest A Remedy For Irregular Periods And Hirsutism

Hi I am 32 years old. I am 155cm tall and weigh 52 kg. I am married and have a son who is 6 years old. History: I have been diagnosed with polycystic ovary since college days. My periods were irregular only once in a while. I conceived my first child after a course of contraceptives of 21days. I had a healthy pregnancy throughout. I have not had any miscarriages till date. Post pregnancy, my periods were regular for almost 3 years. After that it is terribly irregular. I have hirsutism around my chin area for which I had undergone a laser treatment. But it did not work. The hairgrowth still exists. We have been planning for a second child for past 2-3 years but no success. I did an ultrasound which states the previous condition still exist, took several courses of contraceptives, 3 months course of clomid and progestrone. The last treatment ended about 10 months ago. I reduced my weight, and switched to healthy diet plan too. Still my periods are irregular. Its passed 2 months since I had my last one. Could you help me with my problems of Irregular periods Hirsutism and Conception And suggest remedies to these. We are desperate to have a second child

Hello, thanks for writing in. I would like to inform you that polycystic ovaries is a persistent condition which cannot be cured but only modified to improve the quality of life.
I appreciate your dedication towards weight loss and following a diet plan which is worth mentioning here. However, I take note that your periods haven't become regular and even clomid has not helped you conceive. In that case I would like to suggest to to get these tests done before moving further.
1. Oral glucose tolerance test with 75 g of glucose. This is to see how your body handles glucose which is likely to be impaired at times. If the test results show impairment, you may have to take tab.metformin for a long time to improve your glucose tolerance.
2. Tube test also known as HSG ( hysterosalpingogram) to evaluate if your fallopian tubes are open.
3. Serum TSH and free T4, serum prolactin to see if they are contributing for your irregular periods.
4. Your husband's semen analysis.

If all tests turn out normal, I suggest you to go ahead with injections ( gonadotropins) for ovulation induction as clomiphene has failed to help conception. The chances of pregnancy will be higher about 15% per cycle if you try intrauterine insemination along with ovulation induction. Do take folic acid 5 mg daily as a pre conceptional vitamin.
Hirsutism is a result of excess male hormone due to pcos.you can try waxing , depilation to remove hair as existing hair won't disappear. You can only prevent growth of new hair using anti androgenic birth control pills which you can't wait take now. Meet a dermatologist to help you with topical eflornithine cream for hirsutism which is the only FDA approved drug for the purpose. Hope this helps. Good luck!
