Hi

Welcome to HCM

This may be infection of the area ( perineal/ periurethral/prostatic/perianal abscess). Kindly get examined.
Perineal hardness on erection may be normal due to contraction of the muscle in that area to keep the penis erect.

Regards
DR GS
