First of all try to relax.
I would suggest that you get a couple of investigations done to rule out other treatable causes of anxiety
, Thyroid function tests
, blood sugar etc. If all the tests come back as normal, please be reassured that there is nothing majorly wrong with you ,and it is just a case of anxiety.
What happened on July 2011? Did any incident take place at that time which has made you fearful of death.
Regular exercise 30-45 minutes a day 5 days a week is a must. Exercise releases endorphins which will make you feel good.
I would also advise you to sign up for yoga/ relaxation exercises.
If none of the above mentioned measures seem to be helping, I would suggest you visit a psychologist
who might be able to help you by counselling
