Should I Start Taking Insulin If My Blood Sugar Levels Are Controlled?

The summery of blood & urine test reports of my father (a diabetic patient for last 20 years) came on 22nd Nov, 2013 are as below-
Glucose Serum Plasma (F) –135 mg/dL
Glucose Serum Plasma (PP) –245 mg/dL
Randum Urinary Microalbumin – 431.8mg/L (Ref. range less than 30mg/L)
Iron Binding Capacity – 235 µg/dL (Ref. range 250-425µg/L)
Protien Total 24 hrs – 683.2 mg/day (Ref. range upto 150mg/day)
Iron-Serum – 43.9 (Ref. range 59-158µg/dL)
Cystatin C serum by Nephelometry – 1.03 (Ref. range 0.53-0.95mg/L)
All the tests were carried out at Apollo Clinic Indiranagar. Based on these reports the doctor of APOLLO CLINIC, INDIA suggested him medicine on 21st Nov, 2013 as below

Ecosprin (0-1-0) for next 6 month
Orofex XT (0-1-0) for next 6 month
Jenumet 50/1g (1-0-1)
Glador 1 mg (0-1-0)

You also advised to stop Glador after 3 month and start taking insulin LANTUS 400 unit (8unit/day).

Yesterday my father did FGS (Glucose Serum Fasting) & PPGS test again and the results are 101 mg/dL and 122 mg/dL respectively which is normal. However, I have a question If his blood sugar level stays within normal range in next two month then should he start taking insulin after 2 month as suggested by earlier DOCTOR or he can continue with these medicines??

We will be highly obliged if you treat this matter urgent and reply back to us..

Diabetologist 's  Response
dear sir I understand your concern.first of all I am very happy to sugars in control.i will explain you the need of insulin. After seeing blood and urine test I can mahout that dad is having little damage in his kidney.he is having protein leak in urine and cystatin c also elevated.in this kind of conditions even the insulin dose is small it will be of great help in preventing further damage to kidney.to my best knowledge dad should be give drugs like telmesartan, olmesartan, losartan kind of stuff which will help to stop kidney damage...any way his sugar treatment is going on well.my suggestion is to see a nephrologist and find about addition of above stated drugs ,,,,,,regards
