Shaky, dizzyness, blurred vision, elevated heart rate, neck, upper back discomfort, smoker. Reason?

Hi I have not been feeling right for quite sometime, but seems my symptoms are becoming a daily thing.. I feel shakey and flushed, also faint and not with it, my vision gets blurred and my heart rate seems to be elevated, I also have constant neck and upper back discomfort? sometimes worse after smoking but can not bring myself to quit.

In my personal opinion you need to be investigated in detail for the causes. Some history is also required like your age, weight, height, diabetic status, Heart disease, Alcoholic, cough present or not?
The problem your are facing might be due to anemia or some stress or anxiety related.
