Severe Upper Back Pain. MRI Suggests Para Vertebral Muscle Spasm. Treatment Options?

Hi, I have severe upper back pain since 5 months, I have gone thru allopathic, acupuncture , acupressure ,ozonetherapy, physiotherapy and along with other medications, but still there is no improvements. I have a MRI which says Paravertebral Muscle Spasm , and also have associagted dizziness due to this. Pls suggest me.
