Severe Pain, Extra Small Anus, BM, Irritable Bowel Syndrome, Adhesions. Using Enema. Any Suggestion?

Hi I have BM issues /IBS....and adhesions. I cannot pass ANY gas at all is creating pain. Also, I can't "go" on my own, when I push, nothing comes out and I have to use an enema. I am in severe pain but if I go to ER, they say IBS. Been treated for that before. I was born with a extra small Anus. In pain from gas and having to manually get stool out. . I don't know what to do.
