Severe Constipation, Screaming Due To Pain, Miralax Not Working. Castor Oil Can Be Given?

My 4 year old is really constapated this is very normal for him he is on Miralax but it is just not working this time. He has been screaming for 3 days when he has to go because it hurts him so bad. He said his butt and his belly hurts. He did have a bowel movement about 5 hours ago which was not to good of one it was like paste. He is screaming again and saying he has to poop but can not go. Can I give him castor oil?

Thanks for posting your query.
Constipation due to impacted fecolith (dried stools) are very common in children as they generally avoid washroom while playing.
Impacted fecolith at times stuck to colon and difficult to move further in colon.
Miralex may help in fecolith but Miralex (containing Polyethylene Glycol) is contraindicated in children below 12 years.
You should consult with pediatrician/ gastroenterologist and should go for thorough check up.
You can give castor oil along with plenty of water.
If still he can't get relief, you can give 25 ml of liquid paraffin and 25 ml of glycerine (although its unpalatable in taste).
In future, you should teach him to use washroom at proper interval.
