Dear Friend
Welcome to HCM.
The symptoms you are describing are suggestive of "STROKE" especially posterior circulation stroke.
If he has history of diabetes and hypertension , and if he is a smoker , he is already at risk of stroke.
Get him to ER , brain imaging CT/MRI depending on the clinical examination and availability would clear the diagnosis and he will be treated accordingly.
Just don't wait, rush to a hospital NOW.
Good luck.
