Pregnant In Tenth Week, Bleeding Seen With Clots. Did Ultrasound. Advised Bed Rest. What Precautions Can Be Taken?

Hello Doctor, I am in my 10th week pregnancy, last week I had some spots followed by bleeding. ultrasound showed retroplacental clot seen near internal os. Bleeding have been on and off for last 6 days and yesterday there was heavy bleeding(6 pads used) throughtout night with big clots. I was concerned and today did ultrasound again and Doctor have advised of bed rest. Since morning it is stopped. It also shows that posterior placenta low lying reaching inner OS. I am in bed rest but wanted second opinion to avoid being heavily concerned. Is there anything else I shall be worried about to avoid miscarriage. It is not the case till now despite of bleeding but I am worried. One more thing is that despite of bleeding and clots or tissue coming out observed clot(retroplacental) size(22 mm*13mm) remains same in both ultrasound last week and today. I am worried about the clot same dimension despite of bleeding etc. but my doctor mentioned it is reduced.Thanks for your reponse in anticipation.

Per vaginal bleeding and retroplacental hemorrhage are severe coditions especially when the foetal size remains constant or does not change over time.

Most causes of miscarriages during first trimester are caused by chromosomal abnormalities and it is nature's own way of eliminating these foetus that would have defects that would not be compatible with life.

Another possible and common cause as well is progesterone deficiency due to luteal insufficiency. Generally, you should be given proggeserone in addition to strict and absolute bed rest.

However, with reduced foetal size and retroplacetal hemmorrhage, the chances of the foetus staying are very reduced.

You will need to re-do an ultrasound later and if bleeding persist abundantly and if the next ultrasound shows that foetal size is the same or reduced, there will be no option than to terminate the pregnancy.
