Pregnant, Epileptic, On Eptoin, Had Seizures In Sleep On Increase In Dosage. What Should Be Done?

Hi, My wife(26 yrs) is having epilepsy since 2009 and she was taking Valporate tablets. Since Aug 2012, she is taking eptoin 100mg twice every day and she did not have any symptoms. She is pregnant and 4 months old now. We consulted a neuro specialist and they advised to increase the dosage as weight increases during pregnancy. Based on the advise she started taking extra 100mg from today morning and suddenly she got little seizures while sleeping the same day. Kindly advise on what should we do?

Hi,
Your wife is epileptic with 4 months amenorrhea and she being on 200 mg daily and recent increase to 300 mg daily.She requires exactly 5 mg per kg body weight of eptoin.(eg if she is 60 kg then require 300mg per day).She has thrown seizure not because of increase in dose).She requires evaluation and Imaging brain(MRI brain) if not done earlier to rule out structural lesion .She should be on folvite 5 mg daily. Avoid sleep deprivation ,fasting and stress.Ideal drug of choice in pregnancy is levetiracetam ,next is lamotrigine.Consult your neurophysician.Hope this will help you.
