Hello Doctor,

I am a marketing professional (touring job) with bad habit of over eating, eating oily & fatty foods, smoking cigarettes 8-10 per day, drinking alcohol @3X60MLX6 times a month. Recent pathological test reports are given hereunder;S. Cholesterol - 334 mg/dl
S. Triglycerides - 162 mg/dl
S.VLDL-Cholesterol - 54 mg/dl
S.HDL-Cholesterol - 33 mg/dl
S.LDL-Cholesterol -257 mg/dl
Ratio of Cholesterol/HDL - 10.42
P.Glucose Fasting - 102 mg/dl
S.Uric Acid - 8.5 mg/dl

I am really worried to see such report card.

Request suggestion on food; work out, medicine etc. for fastest recovery.

Hi Sanjoy, Welcome to HCM.

I will be able to help you with this. From your lipid profile, the bad cholesterol is high and good cholesterol (HDL) is low. You need to reverse these. I have also noticed that your S. Uric acid, although normal, is on the higher side.

First of all, in order for you to achieve the required target I will strongly advise you to quit smoking and alcohol. If you do want to have alcohol then I will suggest (only) red wine. In regard to your food, you need to restrict all kinds of meat in general since most of these are rich in uric acid. However, occasional chicken and fish are okay but certainly no red meat. Also avoid eggs for few days as the egg-yolk is very rich in cholesterol. As you have mentioned, I also agree with you no fried food or soft drinks or any kind of juices whatsoever is allowed. But do have lots of plain water.

I understand that your job requires you to be away from home and may not be able to be as complaint as you should be to any kind of special diet. The food adjustments I am advising are very simple and are available in most of the restaurants/hotels/grocery stores. At this time salad (lettuce, spinach, cucumber, tomatoes, sprouts etc.) and soup and fruits should be eaten. Also have some walnuts (5 to 6 halved) everyday they are very rich in HDL (good cholesterol). This will help increase it.

Next, endurance exercise such as running, swimming etc. will certainly reduce your cholesterol. You can start with 30 min of brisk walk everyday and then can be gradually increase to running in two weeks or so. Do these diet and exercise changes for couple of months and redo your blood lipid profile.

Most patients I have seen who have adopted this mild lifestyle modification have certainly been benefited. However if for any reason your levels do not improve, I will advise you to consult your primary physician because you may need lipid lowering medications such as low dose statin (Atorvastatin).

Hope this helps.

hello,

you have imapaired lipid profile (and sky high LDL-257,alarming!), current guidelines (and myself too) recommend to give give a trial of Life style modification changes (TLC) for 12 weeks, i.e around 3 months.
TLC - quite smoking completely (may consult psychiatrist as bupropion can help you to certain extent), reduce or stop alcohol. 2 regular packs per week are fairly acceptable .
oily, fatty food, cheese should be reduced to minimum possible, instead replace it with plenty of fruits /vegetable.
and walk for 30-40min atlest 3-4 times a week.
At the ecnd of three months , if lipid profile is is not improved then you shuold start medication like atorvastatin/rosuvastatin in addition to TLC.

you also have raised uric acid, for that you should- Restrict meat/fish/poultry intake,Avoid alcohol and processed foods,Lose weight if overweight,Exercise regularly,Drink 8-10 glasses of water a day

Recommended foods to eat -
Fresh cherries, strawberries, blueberries and other red-blue berries
Bananas,Celery,Tomatoes,Vegetables including cabbage and parsley
Foods high in bromelain (pineapple),Foods high in vitamin C (red cabbage, red bell peppers, tangerines, mandarins, oranges, potatoes),Low-fat dairy products,Complex carbohydrates (breads, cereals),Coffee, tea

Since purines are found in so many foods, it’s important to understand that not all purine-source foods affect the body in the same way. For instance, although purines are found in vegetable sources, these purines don't cause the same amount of uric acid buildup in the body that meat sources do. In fact, fresh fruits and vegetables reduce acid levels in the body ,hence benificial

hope this helps
