Pain In Lower Right Side Stomach. Fever. Appendicitis, Celiac Or Muscle Sprain?

My husband is experiencing excruciating pain in his lower right side right now. Hurts him to move even the slightest, but has some relief when he is still. When I press on his stomach it hurts, but not when I let up. Confused on whether or not this could be his appendix, celiac, or muscle strain. He is also running a low grade fever (99.5).

He need proper clinical examination and few investigations.
Investigations include routine hemogram,urine RE/ME, ultrasound of abdomen and CT Scan of abdomen can be done if needed.
There may be various reasons for pain and it can be excluded after clinical examination and investigations.
Treatment depends accordingly.
