Need Help With Understanding Sliding Scale For Levermir

HI, HOPEFULLY YOU CAN HELP-MY HUSBAND HAS TYPE 2 HE IS CURRENTLY ON HUMALOG AND LEVEMIR FLEX PEN. WE ARE HAVING TROUBLE FINDING A SLIDING SCALE FOR LEVERMIR THAT WE CAN UNDERSTAND. THE DOCTOR GAVE US ONE FOR NOVALOG AND HE HAS CHANGED DOCTORS AND MEDS TO HUMALOG-HE HAD TO HAVE AMBULANCE SAT. AS HE GAVE HIMSELF TO MUCH AND HIS BLOOD SUGAR WAS 35. BELIEVE IT OR NOT, WE REALLY DO HAVE A DOCTOR. DONNA

Dear madam i can understand you concern
usually sliding scale is followed in hospitals to control blood sugar in a short duration
In my clinic usually i will suggest a simplified version of sliding scale to my patients.
to follow sliding scale you need to have a good idea in carb counting.kindly meet a dietitian to understand about carb counting.
checking sugar 2 hrs after food will give you an idea about the insulin what you took before food.
After food sugar are ranging between 140-170 mg/dl that means you are taking adequate dose of pre insulin
in case of low sugars(after food sugar below 120)you may need to reduce the insulin by 2 units
To adjust levemir you need to see fasting blood sugars below 126 mg /dl continue the same dose above 126-140 increase levemir by 3 units until sugars come down to 100-126 mg /dl
