General Surgeon 's  Response
Hi ! Good afternoon. I am Dr Shareef, a general surgeon answering your query.

From the history, it appears that you had a mole for last so many years, and now it is increasing in size, painful, with scab formation. In case it is bleeding much, I would advise you to report to your ER in the nearby hospital. They will first arrest the bleeding possibly with pressure, and then submit the area for an excision biopsy. Depending on the result , further management can be planned.

Wishing you an early recovery. Dr Shaeeef.
