Itching Around Scrotum, Penis, Sore Between Buttocks, Patches Of Swelling. Treatment?

i have been itching for almost a year, and now it seems to be worse around my sctotum and penis at night. now i feel sore in those areas. between my ass cheek also suffers the same fate. at time i itch uncontrollably around my arm , back and legs which leads to small patches of swolleness that eventually leaves lines where the swelling had been.
