General Surgeon 's  Response
If there is discharge from the wound or pain then still surgery is the best option. Recurrence rate is less than 10 percent in experienced hands. further as i have already mentioned leaving the wound to heal by granulation (spontaneouly and not closing or suturing) further improves the success rate

General Surgeon Dr. G. K. Shreedhar's  Response
surgery is gold standard treatment for Pilonidal sinus. Though antibiotic therapy provides temporary healing the recurrence is almost certain. the complete cure can be attained by complete deroofing of the sinus cavity. though the recurrence rate with leaving the sinus cavity open is less, it is more embarrassing till the wound heals. thus i would also recommend removing the hair that grows near the gluteal cleft periodically
