Is Surgery Needed For Enlarged Uterus And Hyperechoic Lesion?

I am a 41yrs aged mother of two kids,Since last few months I observed that my periods are irregular,so I approached to gynaec she asked me to perform a USG,and usg report is uterus is enlarged reveals homogeneous echostructure it meas 98x71mm in size. An hyperechoic lesion seen in body region anteriorly meas 21x12x18mm and posteriorly meas 18x22x12mm in size,Urine cavity empty,endoemtrium normal.
In view of the above report I want to know whether surgery can be avoided or not? pl advice
