Hi! I am a pediatrician. Welcome to Healthcare magic! Let me tell you the answer for your question.
First of all, I will assure you that the present problem is a normal and an expected medical event occurring after vaccination. Since your granddaughter is 4 months old, She must have probably received 3rd dose of Pentavalent vaccine. Low grade fever occur after this vaccination usually lasts for the first 48 hours and it is nothing to worry about.
For fever, i would first recommend you to give tepid sponging for the child with luke warm water. Particular attention should be given to regions like axilla and groin since they are highly vascular structures and help in greater heat loss. And despite this if the fever is persisting, then you can give Tylenol in dose of 100 mg (viz.,4 ml of 125mg/5ml suspension) at 6 hourly intervals.
Hope i have answered your question. Have a good day!
