Is Enlarged Spleen A Cause For Concern?

My friend has been having pain when she lays on her side and she went to the Dr. today and they did an x-ray and told her that her spleen was quite enlarged. They wanted he to go to the city for an ultra-sound immediately. What would be the cause of this and should we be concerned?
