Is MCU And DMSA Testing Necessary In An Infant Suffering From UTI?

Hi, My 7 months old daughter had urinary tract infection. Routine urine analysis showed numerous WBC/pus cells and occassional RBC and the urine culture showed >1,00,000 count of E-coli. Before we received the culture report, the doctor had advised to start on the antibiotics (Sporidex) just to get a head-start. The urine culture report showed that she is resistant to Cephalexin (which is present in Sporidex). But, she was responding well to Sporidex and the doctor suspected the way urine sample collected was a problem (We collected the sample through urine bag). After 7 days course of Sporidex, we had her urine tested. Routine urine analysis showed occasional WBC/Pus cells & culture showed no bacteria. Now, the doctor is suggesting to get MCU & DMSA done. Could you please advise if it is necessary to do MCU & DMSA testing?

Hi.... your doctor is hundred percent correct in suggesting the DMSA scan and MCU scan. Even if I were your pediatrician I would have suggested the same especially keeping in mind the age of the year kid. This scans are done for the following purposes -
DMSA scan to find out if there are any renal scars or inflammation in the Kidneys.
MCU scan is done to find out if there is any upward reflex of urine going on.

If this is the case then the kid will be required to be on prophylactic oral antibiotic till at least 2 years of age.

I concur with your pediatrician and I too suggest that you go ahead with both scans.
