Infant Has Vomiting, Runny And Smelly Stools, Fever. Given Emeset. Maximum Dosage?

Hi doctor What is the maximum dosage of emeset that I can give to my 18 month old baby. She is vomiting and cannot digest anything.not even water .also she has very runny stools almost watery and smelly. She is taking 2.5 ml emeset 3 times a day and grmogyl with that. What can I do.she still in d same situation.also had some fever today

Hello. Thanks for posting on HCM. Your baby is suffering from A/C gastroenteritis. Any baby who comes to me with such symptoms, i do investigate for cholera (stool complete examination and culture) in case of residence in endemic region. Regarding management, i would usually prescribe syp emeset @ 0.1mg/Kg (approx 2.5 ml PO three times a day) and this should be the maximum dose of emeset. Normally, such episodes are viral in origin and antibiotics (Gramogyl) might not be useful. Also i would like any such infant to have the following:
a. syp zinc 20 mg(5 ml)PO OD X 14 days
b. Sporolac Sachet 1 sachet PO BD X 5 days (with milk/Curd)
c. Continue feeding
d. Most importantly, ORS, as per the hydration status, normally @ 50-100 ml per stool, to prevent dehydration. Viral diarrheas, would usually go in 4-5 days. The most important measure in treating such children should be preventing dehydration.

first of all dont panic.what is weight of your baby.medicines are given according to weight.around that age it will approx.10kgs.for a 10 kg of baby emesset will be needed 3.5ml.after giving emesset plz do not give anything to baby to eat or drink as this medicine takes 30-45 to work.after 30 minutes u can give.offer anything in small amount and frequently.maintain proper hydration which is very important.how much fever does he have.if loose stools continue beyond 7 days thn antibiotic is needed otherwise continue with ors,zinc and gramogyl.
