Hi friend,
Welcome to Health Care Magic

Weight is like bank account / The amount in account comes down when you withdraw more (physical work) and you deposit less (eat less). If the weight is picking up, it only means – the exercise is not enough / the food is more / or both.... Probably, he is not doing any physical effort after the attack...
See a good dietician – he has to reduce the quantity of his food and change the quality too. He needs regular exercise.

Episodic rise of pressure may need a separate set of investigations – conditions like Pheochromocytoma have to be excluded...

Discuss with his doctor... The treating doctor alone can suggest based on his assessment of the situation.

Take care
Wishing speedy recovery
God bless
Good luck
