Psychiatrist 's  Response
Hello, welcome to health care magic. I am Dr Alok Sinha, MD Psychiatry from India. I will try my best to answer your query.

I have carefully read your question and I understand your concern.
This condition is most likely fungal infection of prepuce. Due to fungal infection, skin loses its elasticity, develops cracks and fissures and pains a lot. Forget about sexual act, it is even so painful to touch. One should not try to retract it, otherwise the condition may worsen further by developing bigger cracks. Since already you tried to retract it, it might have worsen the situation.
In such cases apart from application of local antifungal cream there may be requirement of oral antifungal medicines, analgesics and anti-inflamatory drugs.
If a person like you comes to my clinic, I advise treatment for both partners (as it may spread to sexual partner). I tell them to maintain sexual abstinence for about two weeks, keep the area dry, avoid rubbing and washing, get blood sugar tested in patient and to take medicines as advised.

You may visit a dermatologist who may prescribe you medicines after examining you. You will be fine soon. I wish you all the best.
