How To Treat Pelvic Floor Dysfunction And Anal Pain Post Delivery?

Hi I gave birth to a 10 lb. 3oz, baby two and a half months ago, and have had severe rectal anal pain and also pelvic pain since the first bowel movement . I was diagnosed with an anal fissure one month later and have been using nifedipine ointment since then to help with the muscle spasms. I was also diagnosed with pelvic floor dysfunction and I have been doing physical therapy for 2 weeks now. Four days ago after a bowel movement where I did have some mild straining, but no constipation, I noticed purple bruising discoloration around the anus, and the pain is very severe. Narcotics are no longer helping this pain, and it is very hard to sit or walk or do anything really. I am concerned about what this could be.
