How To Treat Ingrown Toenail Causing Swelling And Fluid Discharge?

Hello doctor, I have a infected ingrown toenail for about a year and few months I have gone to two different doctors and both have given me anti-biotics and some cream but nothing worked. My toe is still swollen and has fluid running down. I noticed about two days ago my other big toe had become red and painful on the side. This pain is so bareable I can hardly sleep peacefull. I am out of options because the one doctor have told me that I cannot remove the nail due to the swollen and painfull redness. I am worried and cannot take the pain. What would doctor recommend me to do?
