Diabetologist 's  Response
Dear Alok Banerjeem,
Welcome to HCM!
I understand that you are now under treatment for diabetic nephropathy and diabetic retinopathy.
These both are as a result of uncontrolled blood sugars and hypertension.
My suggestions
1. Get a glucometer and do SMBG (Self Monitoring of Blood Glucose) - Fasting, Post prandial and before dinner blood sugars for accurate insulin dose adjustment.
The best treatment for a 20 year diabetic is insulin and the dose needs to be right. Make sure you do not go into hypoglycemias.
2. Get a Home monitoring Blood Presssure instrument (Arm type) and keep monitoring b.p at home. Once in a week go to a nearby general physician to have a manual measurement.
3. Based on the above meet your diabetologist every month for dose adjustment.
I am sure you would have met a nephrologist by now and they would have prescribed a diet chart. Please check urine spot microalbuminuria to assess the extent of protein leakage in urine.
The targets for blood sugar controls are
Fasting - Post prandial - blood pressure 130/80mm of hg

This will help you manage diabetes and renal problems better. You can also opt for an insulin pump if taking insulin daily is troublesome. Discuss with your diabetologist and for any other queries do not hesitate to get back to us.
hope i have answer your query
