Hello,
Welcome to healthcare magic.
From your history and description, I would consider the diagnosis of nodulocystic acne, Acne rosacea and perioral dermatitis.
You could benefit from oral isotretinoin. This is a prescription medicine and has to be taken under the guidance of a dermatologist.
You may also require a course of antibiotics like doxycycline or azithromycin.
In extremely severe cases, a short course of oral steroids also may be warranted.
Do not use any OTC preparations.
You could give gentle wet compresses to remove the scabs and crusts. Apply a gel containing Nadifloxacin twice a day.
I strongly suggest you to consult a dermatologist as such severe acne could lead to deep scars which is very difficult to get rid off in the future.
