How Can Severe Toothache Be Treated?

Hi there i have very poor oral health due to bad phobia 20 years trying to see dentist cant do it many therapys ect just put up with pain and tooth loss for 10 year have very few good teeth left .i have had a tooth ache back molar bottom right for 3 days then suddenly woke up in agony 2 days ago with my face swollen like a golf ball in my cheek .though fear of dentist i have took antibiotics for 3 days and heavy painkiller use ibuprofen and paracetamol to make it bearable but still not slept for 2 days .today the swelling has gone down and pain at about a 6 out of 10 rather than 9 or 10.
I can just about see an abcess on the outerside of the gum and have been salt washing it .its hard ish rather than soft and about a flat grape size .i can only open my mouth about 10 mm and really want this gone but just cant go to a dentist .not an option .please help any help appriciated

I have gone through your query and can understand your concern..

As per your complain it seems that due to long standing infection in you tooth there has been progress of infection below the root tips of the tooth and causing pus formation and ultimately there is formation of a dental abscess..

At times the pus perforates from the bone and accumulates into the soft tissues leading to swelling..

No option other than visiting a Dentist and getting an x ray of tooth done and getting treatment done accordingly can help..
In case if tooth can be saved Root Canal Treatment can help..
In case of tooth cannot be saved extraction is advisable..
Continue doing warm saline gargle and taking painkillers to relieve pain but it will provide only temporary relief..
