How Can Liver Cirrhosis Be Treated?

My brother has been told he has advanced liver disease/cirrhosis. His liver is not processing properly and they've had to drain his one lung of fluids twice this past week. What is his prognosis? He looks terrible and his arms and legs are very skinny. He often has the extended belly, he's had very dark stools and his stomach hurts a lot. We're trying to get him to understand he needs to get his personal information updated. He has been a heavy drinker for many years. He is 57.

Thanks for contacting with your health concern

1. going by the symptoms which you have mentioned [fluid (ascites), dark stools ( probably internal bleeding), liver not functioning properly [pre coma, encephalopathy] it is a case of 'DECOMPENSATED' liver cirrhosis along with Hepato cellular failure

2. Remain under regular supervision of Hepatologist:

i. since the feature of 'stomach hurts' could be indicative either of tuberculous peritonitis, peptic ulcer, pancreatitis

ii. dark stool could be due to ruptured esophageal varices, peptic ulcer, portal hypertensive gastropathy

iii. draining of fluid could be due to evidence of hydrothorax or of pulmonary tuberculosis

3. The information which one needs to get updated is/are:

- Blood examination: Hb%, TLC,DLC, ESR
- stool for occult blood
- Chest x-ray: may show tuberculosis or hydrothorax
- Liver function test: Total proteins [altered albumin: globulin ratio], serum bilirubin [normal to raised], SGOT & SGPT [mild elevation], alkaline phosphatase [slighlty increased], serum cholesterol [low], Prothrombin Time [increased]
- Ultrasonography [or CT scan] of liver, spleen, portal vein [size], any collaterals, and for any free fluid in the abdomen

4. The first measure is to abstain from alcohol which is a hepato toxic agent
. Vitamin D helps prevent liver damage thus green leafy vegetables, eggs, mushrooms, dairy products, apple, orange are good sources
. Eat more of organic foods which are rich in antioxidants and thus allow the liver to detoxify itself.
. avoid food rich in fat/salt
. Take hepatoprotective drugs like: Liv.52, Inositol.
