How Can Erectile Dysfunction With Premature Ejaculation Be Treated?

Hi , iam a 27 year East Indian Man , this past week , i had sex with an 33 year only American Women and lost my VIRGINITY, first i was very attracted towards her .... And even got my erection in office and even thinking about her in my bed ...... but when i started the fore play orgasm ... all went well.... but at last i could not get the erection and then she helped in getting erection ...... i went in and ejaculated in less than 20 secs ..... and lost the erection ... waited for a while and then again went for it ... this time it lasted for 45 secs ..... after that i was unable to get the erection .... So i orally satisfied her ... but i feel embarrassed...... Is it common when a person has sex for the first time? Will minute quantities of premature ejaculation pills will help me? Well she also told that you are really high as I was breathing as fast as I was on the tread mill … now today is my third day after sex and I can feel a little erection on thinking.. ..please suggest …. Age 27 , Wt 160 LBS , height 6 foot, Medical History ( Bilateral Varicole Surgery in 2005, suffered from minute MVP post surgery but I am not on Beta blockers which I took initially )

Thank you for writing to HCM.
I understand your concern for erectile dysfunction and premature ejaculation.
Premature ejaculation is commonly seen during early sexual encounters.
Sometime due to fatigue, lack of mood, situational problem or environmental changes man can fail in sex. It is normal phenomenon. This is called performance anxiety. After such failure when person actively thinks about erection or success of intercourse during sex/masturbation his mind is likely diverted to such thoughts.
For constant erection continuous exciting/romantic/erotic feelings should be there in mind. If mind diverted to concern about erection/ejaculation, ejaculation occurs fast.
When clients with premature ejaculation come to my clinic, I recommend them one or more of the following strategies which often help them.
1. Stop-start technique of behavior therapy.
2. Female on top with active role.
3. Double condom
4. Medicines like Fluoxetine 20 mg day or Paroxetine CR 25 mg per day.
(To be taken under medical supervision only)
5. Kegel exercise also helps in some cases. You can learn the same from internet. (As if try to hold urine intermittently during voiding)
6. Penile squeeze technique applied at the point of ejaculatory inevitability.
7. Applying a local anesthetic ointment to glans penis prior to penetration.
8. Leading partner to orgasm by exploring and stimulating other erogenous zones. (Other than vaginal containment movements only)
9. You should do masturbation before sex (at least 2 hours before). It will make seminal vesicles somewhat empty. So, at the time of sex, semen will take more time to come out.

One can gain ejaculatory control over a period of time by practicing behavior methods mentioned above REGULARLY. Practice makes a man perfect.
