Hello. Welcome to HCM.
I have read your question and understand your concerns.

Your blood sugar levels are not at present in the diabetes range at present. Still go for HBA1c level(3 month average sugar levels) . If it is less than 5.5 than its normal. More than 6.5 is diabetes. 5.5-6.5 is prediabetic stage.

You should consult your doctor and should start medication for cholesterol. Total cholesterol is high and triglycerides are extremely high and need to be controlled on priority basis.

Ideal medication for you is statin +fenofibrate combination(eg. Atorvastatin plus fenofibrate) . Ask your doctor he will understand this.

Do daily brisk walking for 1 hour, avoid oily and fatty foods.

SGPT level is slightly high. No treatment required at present. Just repeat it after 3 weeks.

Thank you..
Have a nice day.
