You have infection which is why sperm dead. Doctor will give antibiotics. After infection goes, sperm may become better and better chance of fertility. Drink plenty of water. Eat plenty of fresh fruits and green leafy vegetables.

Hi well come to HCM.
Its better you consult Doctor who is running fertility clinic since you have only 5 percent sperms that are motile,so without treatment there are hardly any chances
for pregnancy.They may advise you IVF in vitro fertilization. Thanks
