Having Problems Of Indigestion. Blood Tests Done. What Does Findings Suggest?

Hello Doctor,

My mother aged 55 has problems of indigestion and recently hospitalised and discharged for dysentry. The results of blood tests showed increased urea in blood, which led to further diagnosis. RBC count: 10.20 millions/cmm; ESR: 1/2 hr - 20 mm, 1 hr - 41 mm; MCV: 30; MCH: 10.0 (listed only those deviating from reference range). An ultra sound was also done: RT kidney 9.8 X 4.7 cms; LT kidney 10.6 X 4.5 cms. Renal cortical echoes and corticomedullary differentiation is maintained on both sides. Pelvicalyceal system on both sides appear normal. Bladder: normal in contour. No intra luminal echoes are seen.

She has a long history of diabetes. It is said that galvazmet medicine taken for diabetes may affect the kidney function; hence prescribed insulin injections. Moreover, she had her angiogram done in 2010 with variable TMT. 20-30% block was diagnosed and under medications for that (the recent TMT showed no mistake).

Please suggest on the ultrasound findings and further action.

The drug galcavzmet is not responsible for any kidney damage .
You haven't mentioned the exact value of blood urea ?
2nd thing which is very important in functions are serum creatinine level , please mention it?
The size of both kidneys which you mentioned is normal ( it is 10-12 length, 7-9 width, 5-7 thickness)
ESR is high and R B C are very high so for kidney infection may be there as blood urea is high.
Urine analysis also play an important role ,so get in examination.
Consult a urologist and get his opinion.
In my opinion the real reason for damage of kidney is diabetes so try to regulate diabetes.

Hi,
Thanks for using Healthcaremagic,
After going through your history, she has diabetes type II longstanding.
She has suffered acute dysentery which can cause dehydration and some derangement of kidney function that is pre renal Azotemia, which should improve completely once the condition improved. If renal function improves then it is due to dehydration (Pre Renal).
She can develop kidney damage due to longstanding Diabetes, Nephropathy so she should go for micro albumin test, and repeat blood urea and creatinine.
If there is Nephropathy then treatment Insulin is preferable compared to oral ant diabetic drugs?
Please consult an Endocrinologist for proper assessment and further treatment.
